Potato Soup(Crock-Pot) - cooking recipe

Ingredients
    1 (2 lb.) pkg. frozen hash browns
    2 c. water
    2 cubes chicken bouillon
    2 stalks chopped celery
    1 small onion, chopped
    1 medium chopped carrot
    2 cans (10 3/4 oz.) cream of potato soup
    1 pt. half and half
    1 (13 oz.) can evaporated milk
Preparation
    Put all ingredients in a crock-pot except for half and half and simmer on low, for about 6 to 8 hours.
    I start this before I go to work in the morning.
    About 1/2 hour before serving, add half and half.
